Disclosure
Explain the intended intervention, material risks, anticipated benefits and reasonable alternatives in language appropriate to the patient.
Capacity
Capacity is assessed for the specific decision and may vary over time; understanding, appreciation, reasoning and communication are relevant.
Voluntariness
Ensure the patient can decide without coercion, and follow jurisdiction-specific rules for emergencies or legally authorized representatives.
Documentation
Record discussion, questions, relevant findings, decisions, participants and timing accurately; a signed form does not replace a proper conversation.
